At Zamboanga Peninsula Medical Center, Inc., we are committed to supporting the health and well-being of our community. To assist residents of the Zamboanga Peninsula who are in need, we offer financial aid through various government programs. Our goal is to ensure that medical care is accessible to everyone, regardless of their financial situation.

How to Access Assistance

To apply for financial assistance, please visit our Medical Social Service Unit, conveniently located near the information area of the medical center. Our dedicated team is here to guide you through the application process and help you obtain the support you need.

OFFICE OF THE VICE PRESIDENT (OVP) Medical Assistance

FOR ENDORSEMENT, CUT OFF TIME 3:00 PM


Requirements

  • Medical Abstract – (Secure at MSS)

  • Final Billing (Latest)

  • Photocopy of ID (Patient and Representative)

If child without ID present Birth Certificate

  • If without PhilHealth present Indigent Certificate

  • Authorization Letter – If representative is not immediate family

  • Social Case Study Report (To follow upon Endorsement Letter)

- Secure at CSWDO (for residence of Zamboanga City)

- Secure at MSS (for non-residence of Zamboanga City)


NOTE: ONCE REQUIREMENTS ARE COMPLETE PROCEED TO MSS FOR ENDORSEMENT LETTER.


DSWD Medical Assistance (Direct @ Sta. Barbara)

  • Medical Abstract – (Secure at MSS)

  • Final Billing (Latest)

  • Indigent Certificate from your Barangay

  • Authorization Letter – if representative is not immediate family

  • Photocopy of ID (Patient and Representative) If child without ID present Birth Certificate


NOTE: ONCE THE REQUIREMENT ARE COMPLETE PROCEED TO DSWD C.I.U OFFICE LOCATED AT STA. BARBARA, ZAMBOANGA CITY.

CITY MAYOR’S OFFICE Medical Assistance

FOR ENDORSEMENT, CUT OFF TIME 3:00 PM

Requirements

  • Medical Abstract – (Secure at MSS)

  • Final Billing (Latest)

  • Photocopy of ID (Patient and Representative)

If child without ID present Birth Certificate

  • Authorization Letter – If representative is not immediate family

  • Indigent Certificate from your Barangay

  • Social Case Study Report – secure at Barangay Affairs Office City Hall


NOTE: ONCE REQUIREMENT ARE COMPLETE PROCEED TO MSS FOR ENDORSEMENT LETTER.

Medical Assistance to Indigent and Financially Incapacitated Patients (MAIFIP) processing

CUT OFF TIME: 4:00 PM SHARP

Requirements – present to the Medical Social Workers 

  • Guarantee Letter from the proponent (ex. Cong. Olaso, Cong. Dalipe, Cong. Eudela, Cong. Hataman, Cong. Arbison, etc.) 

  • Medical Abstract – (Secure at MSS) 

  • Final Billing (Latest and other assistance deducted) 

  • Photocopy of ID (Patient and Representative) 

  • If child without ID present Birth Certificate
